Mini Menu Assessment and Feedback

Healthy U offers a quick and easy way to get some feedback from a Registered Dietitian! Once you complete this “Mini Menu Assessment” form, you will receive very simple suggestions to help you get on track, and build on your success. These suggestions focus on specific food/beverage types, food/beverage portions, eating schedule, food environment, recipe support, and menu variety. 

(A) Mini Menu Assessment with e-Feedback ($50)

Healthy U e-feedback will be sent to your provided email within 2-5 business days from submitting your form.  

(B) Mini Menu Assessment with 45-minute Phone Feedback ($115)

Healthy U e-feedback will be sent to your provided email within 2-5 business days from submitting your form, followed by a scheduled phone call with Alissa Steinberg to review together in more detail!
